A Note from Superintendent Wilson for all Parents:

Beginning January 12th through March 13th, parents will have the opportunity to participate in the Illinois 5Essentials Parent Survey. This allows you to share your views on important elements of school effectiveness.

On behalf of the Illinois State Board of Education, UChicago Impact will administer the Illinois 5Essentials Survey online. This survey is based on the Five Essentials for School Success, an evidence-based framework developed by the University of Chicago. This framework identifies how school performance on five factors or “essentials” can lead to important student outcomes, including improved attendance and larger test score gains. Those 5Essentials are:

  • Effective Leaders
  • Collaborative Teachers
  • Involved Families
  • Supportive Environment
  • Ambitious Instruction
The parent survey takes approximately 10 minutes. Parents may skip any questions they do not wish to answer; however, they must click "Submit Survey" once they have reached the end in order for responses to be counted towards their school's response rate and included in the parent report.

To complete the parent survey:
  • Visit https://survey.5-essentials.org/illinois/ and select Parent Survey
  • Select the necessary language to complete the survey
  • Enter requested demographic information
  • Advance to survey questions
  • Answer all applicable questions
  • Click Submit Survey

For more information about the Illinois 5Essentials Survey, or to view 2014 Reports, please visit https://illinois.5-essentials.org. If you have any questions about the survey, please contact 5Essentials Customer Support at 1-866-440-1874 or  This e-mail address is being protected from spam bots, you need JavaScript enabled to view it .
